Athletic Physical Forms for 2024-25
*This is a reminder for all junior high and high school athletes. A current physical must be on file with the high school office BEFORE the first day of practice for the 2024-25 school year.  Please turn those in to the coach or directly to the high school office.

Consent to Treat Form for Parkview Sports Management  2023-2024
*This form must be completed and turned in PRIOR to a student-athlete being able to receive any healthcare treatment at North Central by the athletic trainer.
